The narrative centers on a brilliant doctor, known as (Kairi Narita), who seeks absolute revenge after his wife is brutally raped and murdered by the sadistic son of a powerful businessman. The plan he devises is as outlandish as it is horrific: he purchases a young woman, Mayumi (Asami), a former drug addict, and subjected her to grueling physical and mental training to become a living weapon.

The "Gun Woman" title becomes literal in the final act. To bypass maximum security at an underground facility, the doctor surgically implants gun components into Mayumi’s body. Her mission is a suicide run; she must cut the parts out of her own flesh, assemble the firearm, and assassinate the target before she bleeds to death.
