According to David Draiman, the song was inspired by the suicide of an ex-girlfriend when he was 16 years old. In interviews, Draiman has explained that his girlfriend struggled with a severe drug addiction (heroin), which she had also introduced to him. After Draiman ended the relationship due to the destructive nature of their dynamic, she took her own life.

The lyrics of "Inside the Fire" are written from the perspective of the Devil tempting a grieving survivor.
